SARS-CoV-2 or COVID 19

The World Health Organisation (WHO) was informed about a cluster of pneumonia cases of unknown aetiology in Wuhan City, Hubei Province, China on 31 December 2019. On the 7th of January 2020, a Novel Coronavirus (2019-nCoV) was identified as the causative virus by the Chinese authorities. The Novel Coronavirus is formally named SARS-CoV-2. The disease caused by the virus is now referred to as COVID-19.​ Since then, the virus has spread to more than 100 countries.

SARS-CoV-2 is an enveloped, positive sense, single-stranded ribonucleic acid (RNA) virus with a 30-kb genome. The virus has an RNA proofreading mechanism keeping the mutation rate relatively low. The genome encodes for non-structural proteins and four structural proteins. SARS-CoV-2 is the seventh coronavirus identified that is known to infect humans.

The clinical presentation of SARS-CoV-2 infection can range from asymptomatic infection to severe disease. Mortality rates differ per country. Early laboratory diagnosis of a SARS-CoV-2 infection can aid clinical management and outbreak control.

COVID 19 PCR BASED DIAGNOSTIC TESTING

The gold standard for detecting SARS-CoV-2 is a polymerase chain reaction (PCR). This tests for the genetic material of the virus, and is performed most commonly on a swab taken from the nose and throat, or from sputum (mucus from the lungs) in unwell patients.

Quantitative RT-PCR monitors the amplification of the virus during the course of the PCR (i.e., in real time), not at its end. The method is used to estimate the viral load in a patient. Should the detection exceed a certain threshold, the patient will be positive for SARS-CoV-2.
